Vanished after waking up and flying when getting out of a car

I experienced this 2 times today and it wasn't the first time.
I logged onto the server again and was in vanish mode.
I am totally sure that I haven't been when I logged off but I had been seconds before that.

The same with the car. I think I even was vanished when getting in the car, turned vanish off then and when stepping out
of the car I wasn't vanished but flying.

I removed oxide permission vanish.permanent 
seemed to work for me

I don't use vanish.permanent by standard.

Have you double-checked that? Because it sounds very much like that permission is assigned.

oxide.show perm vanish.permanent

Yep. No players with that permission and no group too.

It's been happening to me too. It is strange. I log off actively checking to make sure I am not in vanish. But the next time I log in, I vanish. It doesn't seem to activate until waking up though. I have another admin that I can see laying in our base, but as soon as they log on, they vanish. 
and I'm showing "No players with this permission No groups with this permission" when looking at vanish.permanent. 

I just changed
If a player was vanished on disconnection keep them vanished on reconnect 
to false, just in case it is not registering right when i log off. Maybe that will help

This should be fixed in the latest patch <3

It's not fixed. Just had that in a boat. Mounted in vanished mode. Deactivated Vanish Mode and when unmounted I was flying.
