This may have already been mentioned, but it's been established that vanilla will leak held entities on server restarts. For example, for items that are added to container entities which have saving disabled, such as various loot containers. However, this will likely not leak more than a few hundred per restart, so if you have a large quantity of leaked held entities, it's likely that plugins are contributing.

i see this is from a year ago, i just ran performace manager and found a ton of handheld entities just sitting there, deleted about 10k of them. any ideas how to trouble shoot which is causing it?

Don't delete them. You can break existing items and players will be not able to use them. In every case its individual, you need to hire someone like me to look into your plugins
Someone asks about this problem every few months. Would be useful if there was a thread or guide that could teach people to avoid this in their plugins.

What symptoms are you seeing?
Question for you guys. Im new to running rust server. First one. Map size is 3050, about 20 players so far, and about 35 plugins. I do not use kits. My entity count is 136k. Is this to high for server that is only one week old? Thanks for any input.
It's hard to say. A fresh wiped 3k map is going to have something like 30k entities so you're around 100k over that in one week.
Player buildings can make up a huge part of entity count so balancing gather rates is important.
If you think the building-boom is more or less over on your server then entity count will probably rise at a slower rate now than it did in week one.
Some servers track raids and player activity and clean up disused bases after a period of time but whether that's necessary remains to be seen.
If this is your first wipe I suppose just keep an eye on entity count over time, and server performance.
Ultimately if performance is good, you don't really need to worry about it.
all my plugins are up to dates, the symptones are lags, server fps dropdown, lots of ents amount, and always have more than 30-40k planners at the end of wipe. And always had this, a stupid amount of planners
870k entitys 5-10 players normally not over 250k
My entitys nearly hit 900k in a normally 250k server added an extra 24 gb of ram to contend with it someone please help me find what it is
I had similar problem this month, 500k fuel_storage entities and also around 10k planner.
Is there any downside to mass-deleting planner entities? Obviously mass-deleting fuel_storage would cause a lot of issues with vehicles etc.

Mass deleting planners will break many legitimate held entities, causing items to no longer work. Any deletion of planners should be done selectively (by plugin logic) to verify that they are no longer able to be used by players (i.e., not in containers).
