Warning doesn't seem to be working

I have Clear Offense After... set at 3
Offenses Count to Mute set at 3
and Warn set to True

I don't see any warnings for committed offenses.

can supply your config . 
and posible version you use ....
so i can see whats wrong

just tested my side with your settings and did get warnings and muted at 3....... maybe your lang file corupted .... .... best i can ask you now is to delete all . lang config and plugin and start from scratch and see what happens ... just back up your word list
