Pretty sure that's what it does, UNLESS you have assigned the exclude permission. People with that permission WILL NOT auto scream.

What do you mean by automated? Scream-on-demand lets the player scream regardless of player's state. The scream is automatic when wounded unless configured otherwise.

i guess my config is wrong because it only always me to do it on demand
