Never Mind I found the issue (Extra Loot) oups

That is 1 plugin listed on my Known Issues doc page if you read it..

Yeah, that Extra Loot plugin is fucked in general & doesn't handle the items correctly

( It breaks the item data preventing the instances from stacking )

so the only thing you can do to fix your server is to just not use Extra Loot & wipe the server of all game items.
