Could you add chance to get effect uppon taking pill
something similar to what plugin "Mushroom Effect by supreme" uses 

config chance to get sick 0%to100%
or 
min amount after getting sick
or 
most realistic and perfect would be time based 
so if more than 3 pills taken in 120sec period then its 100% getting sick
