The only way that could happen is if you have a plugin that upgrades or changes the workbench and doesn't attach it to the building. If the workbench is not part of the building this plugin doesn't detect it and will look like it's not working.

Not sure, no issues since one of the servers wiped but it seems it doesnt like existing workbenches. We dropped it on 3 servers and all 3 had the same issue. So not sure what happened. We don't have any plugins that don't attach it to the building. Even the upgrade workbenches attaches it to the building. 

The Upgrade Workbench attaches it to buildings on the latest version. I took over the plugin to fix a bug the previous versions had. If you're on a previous version before 2.0.0 then it will not work with this plugin.

Nah we have had a custom one for a long time now and we already checked and it attaches it to the building. Like the server that just wiped 0 issues have occured at all. Only issues occured when I dropped in on a server thats been up already for a wipe. I honestly have no idea what happened.
