there should be a check to identify item damage & NOT let it stack into a pile of existing items.
Becasue if it can.. the Metadata associated with the item is "stripped" and it just becomes a "normal" item

That would pre suppose and indepth worknig knowlege of the code base... for both the plugin & RUST,
your comment shows a distinct lack of understanding of the type of people running servers.
Besides which it is not just items with health bars..... and indeed..... up until a few months ago the RUST codebase had exactly the SAME issues on some "health bar" items... WITHOUT the needed inclusion of "oxide" or indeed this plugin.
but I suppose in your eyes that would be "my error" as well.

Was that item fully repaired in a workbench? As in, the green health bar was full (ignoring the red)? Items with different green health bars shouldn't be affected because Facepunch had fixed that a while ago. This is new to me however, I'll look into it.

Yes, item was fully repair to red bar in a workbench. Repaired items with different level of red bar can be stack. Then you can divide them to items without red bars.
