I have a hardcore server that is heavily modded and the problem is that in some instances, players log off the server, then can no rejoin again until I wipe the server. I made a clone of the server, with save file, plugins, etc, and the player could join the cloned server. The fog of war was reset for them on the test server, so that is probably why.
If they are dead in-game, they will get

Anyone have any ideas on leads which direction to look?
I just removed LImit Entities plugin in hopes that might be the cause, but I will have to wait to see if someone else gets this issue. Out of hundreds of players about 5-10 get affected, so it is not widespread for all users on the server.
