Weapons are not usable sometimes when reloading. I currently have no weapon plugins.

When did this issue start happening for you? Do you use any plugin that claims to clean up leaked or broken items? Are you using the Backpacks plugins and were these items previously placed in a backpack?

I do have orange items cleaner that could possibly be doing that.

Yeah that's known to cause it, in particular with Backpacks. There are alternate item cleaner plugins floating around that handle Backpacks better.

Somebody in the uMod Discord also recently discovered a significant vanilla cause of held item leaks, which is loot crates that don't have saving enabled. Whenever the server restarts, the items' held entities are saved indefinitely even though the loot crates are not. Some day somebody will make a proper fix for this, so item cleaner plugins like the one you mentioned won't be necessary.
