Noticed that on a custom map the lag gets progressively worse.  Unloaded all plugins with the same result.  Never have I ever had a problem like this before.  When turning, there's so much blur you can't shoot straight.  If anyone has any ideas, I'd love to read them.
I can disconnect and reconnect and it fixes it for a few minutes before going back to the blur/lag.
Map is only 2k.
