Following the banid “steamid” “racism” command, it only says the player was banned but does not include the reason.  When the banned player attempts to connect, it still states "no reason".  I'm using the command shown on CorrosionHour.

Is there a way to actually include the reason and have it shown?

Use   ban “steamid” “racism”

banid has a third parameter, name, because a user that is offline, or never logged before, can be banned with this command:

   

banid "steamid" "name" "reason"
