This happens sometimes on my server. The thing is for some people it works fine. But obviously i'm missing out on players when this is happening and usually have to hear it from people that go to the trouble of mentioning it.

Any idea how this could be fixed? Of course a restart usually fixes it, but I need to find the cause. Because this error is very dangerous because it makes you miss out on players without noticing it yourself if its working fine for you.
