Hi, I would like to know why there are so many mistakes?
 
Without seeing the full error (above what you provided), it would be hard to know what the issue is exactly, but it's related to the hook calling for that OnUserApprove hook in Oxide itself.
this is the whole log right after the player is thrown out for whitelist
