I am getting reports that some players are seeing the symbol sometimes? Any ideas what could be causing this?

Do you use FauxAdmin/Godmode?

Nope not at all, just normal oxide groups, only 1 admin group, VIPs and default, and random players seems to be getting it, most when they are wounded, it doesn't do anything, they don't have Godmode, but it does show up for them
