the only way this can happen is with a bow or no weapon at all
I tried and your config works fine so it's not that. if they have no items then another plugin is breaking it. most commonly its a kits plugin with a broken IsKit API so the npc spawns with no items at all because the plugin said the kit exists when it doesn't. Kits plugin from uMod works fine.
regardless, I had to deal with this in Raidable Bases already and created a workaround for it. it will also allow you to use crossbows, hunting bows, compound bows or any weapon with no muzzlepoint transform object.
