After the Nov. 4th Rust Update, bank is giving a "missing loot panels" error and any items that players try to place in it disappear.

I couldn't reproduce this. Also, from looking at the code, I don't see the tell tale signs that would cause this which we found in other plugins. Can you share your config?

Apparently, Bank is working now.  I updated a few other plugins as soon as updates became available so perhaps that... helped somehow?  Sorry for the false alarm.
